During the first and second peak of the coronavirus disease-19 (COVID-19) pandemic, an increased incidence of Out-of-Hospital Cardiac Arrest (OHCA) was observed, especially in the most affected countries like Indonesia. This increase in OHCA was explained by both infection-related and pandemic-related causes. The most common infection-related causes are respiratory failure, pulmonary embolism, myocarditis, myocardial injury, myocardial infarction due to thrombosis-promoting conditions, rapid progression of arrhythmias due to SARS-CoV-2 infection and/or side effects of drug.
Burden of COVID-19 Pandemics
On the other hand, the fear of nosocomial infections prevents patients from using the emergency medical system. On-demand and reduced visits to outpatient clinics were the most common pandemic-related causes leading to exacerbations of time-related illnesses at home. However, increased OHCA incidence has not been observed in all countries, especially in developing countries, probably due to different pandemic burdens. In addition, the correlation between OHCA incidence and COVID-19 cases has been proposed in the countries most affected during both the ascending and descending phases of the first pandemic wave.
